The process step by step

Anesthesia: General anesthesia (sometimes local + sedation) · 1-2 hours (depending on the scope of the surgery)

    Initial consultation and discussion of expectations(30-45 minutes)

    The plastic surgeon listens to the patient's concerns about the appearance of the chest, gives an initial assessment of whether the tissue is glandular or fat-based, and explains the expected outcome.

    Medical examination and cause investigation(1 day)

    Hormonal tests (for example, testosterone, estrogen) and, if needed, ultrasound are performed to investigate the cause of the gynecomastia (hormonal, drug-induced, idiopathic). Blood tests and a general health assessment for anesthesia are carried out.

    Surgical planning(1 session)

    Depending on tissue composition, liposuction (for fat-based cases), surgical removal of glandular tissue, or a combination of both is planned. Incision location and the expected visual outcome are finalized together with the patient.

    Anesthesia(20-30 minutes)

    Depending on the scope of the surgery, general anesthesia or local anesthesia combined with sedation is administered. The patient feels no pain throughout the procedure.

    Surgery(1-2 hours)

    According to the chosen technique, excess fat is removed by liposuction and/or glandular tissue is surgically removed through a small incision. At the end of surgery, a drain tube (if needed) and a supportive compression garment are applied.

    Initial recovery and observation(12-24 hours)

    The patient is monitored in the recovery room and is usually discharged home the same day or after one night of observation. A medication schedule for pain and swelling is prescribed.

Recovery process

Week 1

Swelling, bruising, and tenderness in the chest are most pronounced during this period. The compression garment is worn continuously. Mild to moderate pain and a feeling of tightness are normal.

Restriction: Heavy physical activity, movements requiring arm exertion, and sleeping face down are restricted. Continuous wear of the compression garment for the indicated period is essential.

Weeks 2-4

The main swelling subsides and the chest contour becomes more defined. By the end of this period, most patients can return to office-type work and light daily activity.

Restriction: Chest-focused exercise (bench press, push-ups) and contact sports are not recommended during this time.

1-3 months

Most of the visible swelling resolves and the chest contour flattens, taking on a more masculine shape. The skin gradually adapts to the new contour.

Restriction: It is recommended to get the surgeon's approval before fully returning to heavy shoulder/chest exercise.

3-6 months

The fine swelling fully resolves, the skin tightens, and the final chest shape emerges — this is the realistic timeframe needed for the complete result of gynecomastia surgery to appear.

Restriction: No special restrictions; sports and daily life are fully normal.

Can gynecomastia be resolved with exercise and diet?

If the excess volume is mainly fat tissue, weight loss and exercise may provide some reduction. However, when true glandular tissue enlargement is present, this tissue does not decrease with exercise and diet — it can only be removed surgically.

How long-lasting is the result?

The removed glandular tissue and fat cells do not return after surgery, so the result is generally considered permanent. However, significant weight gain, steroid use, or a persisting hormonal problem may cause the tissue to regrow somewhat.

Are there any visible scars after the operation?

The area treated with liposuction leaves a very small, nearly invisible scar. When surgical removal of glandular tissue is required, a thin scar usually remains around the nipple, which fades over time.

From what age can gynecomastia surgery be performed?

Gynecomastia that appears during puberty often resolves on its own within 1-2 years, so surgery is usually recommended after physical development is complete (around age 18) and the condition has stabilized. Persistent cases at an earlier age that cause psychological distress can be assessed separately.

What is the best season for gynecomastia surgery?

Medically, a specific season is not a requirement; the operation is performed safely year-round. Some patients prefer the winter months for comfortable wear of the compression garment, or the summer months to feel comfortable shirtless.

Can gynecomastia recur?

Surgically removed tissue does not return, but if the underlying cause is not addressed (for example, continued steroid use, an untreated hormonal problem, significant weight gain), new tissue may form again to some degree. This risk is reduced by properly investigating the cause before surgery.

What is the difference between liposuction and surgical removal?

Liposuction is used only to reduce fat tissue and requires a smaller incision. Glandular tissue cannot be removed by liposuction — it must be surgically excised through a small incision. In most cases, both techniques are combined in a single operation, since gynecomastia usually involves a mix of fat and glandular tissue.
